This online PDF form can be used to request a main stage performance time. This form can be filled online. Most browsers support fillable PDF forms. If your browser does not support fillable forms, download a copy of the file and fill it out manually.

Request Procedure

1. Access the Performance Request Form (fillable PDF).
2. Complete the form online using your browser.
3. Electronically sign and date the form by entering you name and date in the fields provided. It is not necessary to physically sign the form.
4. Download and save the completed form as a PDF file.
5. Note: If you cannot complete the form online, download a copy of the form, fill it out manually, and scan the form, saving it as a PDF file.
6. Email the PDF file to japan@himele.org.
   
7. A verification of your request will be sent to the email address you provide on the form.
